REQUIRED DOCUMENTS TO UPLOAD

Once you have completed your online registration and payment, please download and print copies of the forms listed below. You can then upload copies of the completed forms and additional required documents using the form below. Copies must be uploaded  by July 1st. Please keep hard copies of each document with the original signatures; packets will be collected at future parent meetings, events, and practices (instructions will be sent out at a later date).

Other Documents Needed:

  • School Enrollment Verification: report card or other printout showing enrollment year and grade (for cheer, it is only required for 12U and 14U)
  • Government ID: must be a passport, passport card, Real ID, California ID, or Military ID
