AAU DOCUMENTS
REQUIRED DOCUMENTS
Once you have completed your online registration and payment, please download and print copies of the forms listed below. You can upload copies of the completed forms and additional required documents starting March 15th. Hard copies of the originals must be turned in to the chapter AD starting April 1st. We will send out reminders with additional information once document uploads and paperwork submission open. Physicals must be dated after January 1, 2026.
Other Documents Needed:
- School Enrollment Verification: report card or other printout showing enrollment year and grade (for cheer, it is only required for 12U and 14U)
- Government ID: must be a passport, passport card, Real ID, California ID, or Military ID. CLICK HERE for more information on the AAU ID guidelines that were established in 2021.
