REQUIRED DOCUMENTS
Once you have completed your online registration and payment, please download and print copies of the forms listed below. You can upload copies of the completed forms and additional required documents starting April 1st. Hard copies of the originals must be turned in to your team AD starting April 15th. We will send out reminders with document upload instructions.
Other Documents Needed:
- School Enrollment Verification: report card or other printout showing enrollment year and grade (for cheer, it is only required for 12U and 14U)
- Government ID: must be a passport, passport card, Real ID, California ID, or Military ID
